Symptoms of Pancreatic Cysts
Most pancreatic cysts do not cause symptoms and are found incidentally on imaging done for other reasons. When symptoms do occur, they may include:
- Abdominal pain (most common symptom when present)
- Nausea or vomiting
- Pancreatitis (acute inflammation)
- Yellowing of the skin or eyes (jaundice), if a cyst blocks the bile duct
- Weight loss
- A noticeable abdominal mass (rare and usually seen only with large cysts)

How are Pancreatic Cysts Diagnosed?

- CT or MRI with MRCP (magnetic resonance cholangiopancreatography): These imaging studies are usually the first step. They help determine the cyst’s size, location, appearance, and whether it involves the pancreatic duct.
- Endoscopic ultrasound (EUS): This test provides detailed images of the pancreas and may allow your doctor to collect a small fluid sample from the cyst for further analysis.
How are Pancreatic Cysts Treated?
Treatment depends on the type of cyst and its risk of becoming cancerous.
Monitoring (Surveillance)
Most pancreatic cysts are low-risk and can be safely monitored over time. Small cysts (typically less than 3 cm) that are not causing symptoms and do not have concerning features are often followed with periodic imaging rather than immediate treatment.
- MRI is the preferred imaging test for monitoring pancreatic cysts
- Follow-up imaging intervals vary by cyst type and size, typically ranging from 6 months to 2 years
- If a cyst remains stable for several years and you remain in good health, your doctor may discuss whether continued monitoring is still needed
- Monitoring may also be adjusted based on your age and overall health status
Surgery
Surgery may be recommended for cysts with high-risk features, which include:
- A solid nodule or growth within the cyst
- Significant widening of the pancreatic duct
- Jaundice (yellowing of the skin or eyes) caused by the cyst
- Biopsy results showing precancerous or cancerous cells
Surgery may also be considered for cysts causing symptoms (such as pain or pancreatitis) or for certain intermediate-risk cysts, depending on individual factors and patient preference.
